1. Preparation for Surgery
<ol>
	<li>Allow a soft diet one day before surgery, with nothing by mouth after midnight before the surgery. 
	Note: Do not insert a nasogastric tube before and during the surgery. This protocol does not use any tube to deflate the stomach. Instead of that, the intra-gastric gas can be aspirated by laparoscopic puncture needle if the stomach is distended causing limited view for laparoscopic surgery.</li>
	<li>After placing the patient on the operating table, administer the induction agent (Thiopental Sodium, 100 mg).</li>
	<li>After intravenous injection of a muscle relaxant (Rocuronium bromide, 1.0 mg/kg), insert the endotracheal tube through the oral cavity.</li>
	<li>Start the anesthetic gas (Sevoflurane, 2 - 3%) via the endotracheal tube, then monitor the patient for stability during surgery.</li>
</ol>
2. Surgical Procedure before Intraoperative Gastroscopy
<ol>
	<li>Place the patient in reverse Trendelenburg position, and stand at the right side of the patient.</li>
	<li>Make 10 mm incision at the infraumbilical area, and insert the first trocar into the abdominal cavity.</li>
	<li>As soon as the trocar is inserted, connect the gas tube for carbon dioxide with the trocar to create a pneumoperitoneum of 15 - 18 mm Hg.</li>
	<li>Insert a rigid 30&#176; laparoscope through the trocar, and explore the abdominal cavity on a monitor connected with the laparoscopic system.</li>
	<li>Insert 4 additional trocars (one 12 mm and three 5 mm) as working ports. Insert 12 mm trocar at the right upper side of the umbilicus. Insert other three 5 mm trocars at left upper side of the umbilicus and both upper outer quadrants of the abdomen.</li>
	<li>Dissect the gastrocolic ligament along the transverse colon toward the inferior pole of the spleen using ultrasonic endoscopic scissors.</li>
	<li>Ligate the left gastroepiploic artery and vein originating from splenic vessels at the root with a laparoscopic clip and resect them using ultrasonic endoscopic scissors, after exposure of the superior border of the pancreatic tail.</li>
	<li>Using ultrasonic endoscopic scissors, dissect the peripyloric lymph nodes around the pylorus and the head of the pancreas, and resect the right gastroepiploic vessels at their origin.</li>
	<li>Before resection of the first portion of the duodenum, clamp the duodenum with a laparoscopic clamp to prevent migration of gas into the small bowel during the gastroscopic procedure (Figure 1).</li>
</ol>
3. Intraoperative Gastroscopy
<ol>
	<li>Move to the left side of the patient&#39;s head to perform intraoperative gastroscopy.</li>
	<li>Insert a gum shield to protect the teeth and gums during intraoperative gastroscopy.</li>
	<li>Turn the head of the patient to the left side for insertion of the gastroscope.</li>
	<li>Insert the gastroscope into the stomach carefully through the mouth and the esophagus, to avoid injury to the mucosa of the oral cavity, the esophagus, and the stomach.</li>
	<li>Insufflate the stomach by injecting gas through the gastroscope to locate the primary lesion. 
	Note: Most gastroscopes have a specific button to inject gas into the bowel during the procedure.</li>
	<li>Approximate the distance between the proximal margin of the tumor and the esophagogastric junction using the scale marked on the gastroscope (Figure 2), and perform total gastrectomy (as described previously) if the distance is too short to salvage the proximal stomach.10</li>
	<li>If total gastrectomy is not necessary, insert an endoscopic injector through a small hole of the gastroscope, and inject 2 - 3 cc of indigo carmine into the gastric wall using an endoscopic injector at an area 2 - 3 cm proximal from the tumor margin, avoiding intra- or extraluminal leakage (Figure 3A).
	<ol>
		<li>Insert the needle of the injector into the gastric wall obliquely to avoid the perforation of gastric wall by the needle. In addition, inject indigo carmine very slowly to identify intraluminal leakage during injection.</li>
	</ol>
	</li>
	<li>After dye injection, aspirate inflation gas through the gastroscope to make the laparoscopic procedure easier. 
	Note: Most gastroscopes have a specific button to aspirate inflation gas during the procedure.</li>
</ol>
4. Procedure after Intraoperative Gastroscopy
<ol>
	<li>Return to the right side of the patient after the gastroscopic procedure.</li>
	<li>In the laparoscopic view, confirm the proximal portion of the tumor by identifying serosal staining by blue dye (Figure 3B).</li>
	<li>Mark the stained portion to be resected with an endoscopic clip, avoiding involvement of tumor at the resection margin (Figure 4A). 
	Note: We recommend performing this procedure within 5 - 10 min after surgery begins, because the dye can sometimes be washed out during subsequent procedures.</li>
	<li>If the dye cannot be detected in the serosa, immediately re-inject the dye under gastroscopic visualization.</li>
</ol>
5. Procedure after Intraoperative Gastroscopy
<ol>
	<li>Resect the duodenum, and perform other procedures for lymph node dissection according to the Japanese treatment guidelines.11</li>
	<li>After lymph node dissection, resect the stomach at the proper location at least 3 cm proximal from the tumor margin, guided by the marking clip, using 2 endoscopic staplers.</li>
	<li>As soon as the distal stomach is resected, insert the specimen into an endoscopic bag and withdraw it from the abdominal cavity through a 2 - 3 cm extension of the infraumbilical trocar site.</li>
	<li>Send the tissue from the proximal margin to pathology for intraoperative histological evaluation.</li>
	<li>After confirmation of tumor-free tissue in the proximal portion of the resected stomach, perform an anastomosis between the remaining stomach and the proximal jejunum with intracorporeal linear staplers (Figure 4B).</li>

In this protocol, we introduced a procedure for intraoperative gastroscopy to accurately and reliably identify tumor location in the middle third of the stomach. We were able to successfully perform distal subtotal gastrectomy based on the findings of intraoperative gastroscopy.
To date, various methods have been applied to identify the location of early gastric cancer in the laparoscopic view. Most common methods used preoperative gastroscopic clipping, and surgeons could detect the location of clips using laparoscopic ultrasonography or portable plain radiography.7,9 These techniques required additional preoperative gastroscopy one or two days before surgery. Laparoscopic ultrasonography for intraoperative detection of clips requires specific instruments and skills and involves subjective interpretation. Another option, intraoperative portable radiology, can show only a two-dimensional view; additional exposure to radiation can be harmful to patients. Recently, Jeong and colleague reported gastroscopic autologous blood tattooing before surgery.8 They reported that this procedure also requires preoperative gastroscopy one day before surgery, and consequently is inconvenient for patients. Another disadvantage of this procedure is that it is impossible to recognize incorrect injection into the gastric wall before surgery.
Unlike other procedures, we performed additional gastroscopy while patients were under general anesthesia during surgery. Therefore, patients enrolled in the present study avoided the inconvenience of additional preoperative gastroscopy. The dye injected into the muscle layer was easily detected during surgery, because we examined the serosa within 5 to 10 min after injection, and immediately clipped the center of the stained serosa before the dye spread. The exact tumor location was detected in all patients with our procedure.
The critical step of our procedure is injection of the blue dye into the gastric wall without leakage into the extraluminal area. When more commonly used dyes, such as methylene blue, leak into the intraperitoneal region, there is concern for the occurrence of postoperative adhesions, methemoglobinemia, and pulmonary edema by chromopertubation. In our protocol, indigo carmine is used instead of methylene blue, because it is a relatively nontoxic, inexpensive dye. Even so, the leakage of dye can make precise localization of tumors difficult. Therefore, it is recommended that a surgeon experienced in gastroscopic procedures, such as endoscopic injection, perform this procedure.
When blue dye and a specific instrument like an endoscopic injector are not available, our procedure can be modified. As soon as the operating surgeon inserts the gastroscope and inflates the stomach, the assistant surgeon compresses the area where the primary tumor is expected to be located, enabling the operating surgeon to identify the distance between the location of the primary tumor and the compressed lesion. Through communication between the operating and the assistant surgeon, the tumor can be precisely localized.
Several limitations of our technique for tumor localization should be considered. First, intraoperative gastroscopy may require a learning curve for specific endoscopic skills. One report suggested that surgeons would overcome the learning curve of gastroscopic procedures after about 80-90 cases.12 The surgeon who participated in this study had sufficient experience (more than 400 cases) to overcome the learning curve. Sometimes, intraoperative gastroscopy may be difficult due to the positioning of patients under general anesthesia. If the anesthesiologist extends the jaw of the patient, the gastroscope can be easily passed through the throat and into the esophagus. Second, specific devices for this procedure, like the gastroscopic system, should be ready in the operating room. However, several recently suggested procedures for gastric resection also require use of an intraoperative gastroscope.13,14 In the future, intraoperative gastroscopy will be more common in gastric surgery, and the gastroscopic system will be essential in the operating room Third, tumor margins were only determined by endoscopic findings in this protocol. Sometimes, the margin of superficial spreading tumors can be unclear, even though an elevated and discolored lesion can be assumed to be a tumor. Recent evolving techniques, such as infrared endoscopy or molecular in vivo imaging, may verify the tumor margin; if applied to intraoperative gastroscopy,15,16 the procedure in this protocol can be more accurate and useful.
In conclusion, intraoperative gastroscopy for tumor localization is an accurate and tolerated method for gastric cancer patients undergoing totally laparoscopic distal gastrectomy.

Laparoscopic surgery has been become the treatment of choice for early gastric cancer in East Asian countries, including Korea and Japan.1 The advantage of this surgery has been well demonstrated in several clinical trials for early gastric cancer (EGC).2-4 Most of the procedures in these trials were accomplished by laparoscopy, but identification of the tumor location, resection of the stomach, including the primary tumor, and reconstruction were performed via minilaparotomy. Therefore, surgery requiring minilaparotomy has been labeled &#34;laparoscopy-assisted gastrectomy.&#34;
Recently, however, surgical procedures have evolved to minimize trauma, improving the postoperative quality of life for patients. Since this concept has also been applied in laparoscopic surgery for gastric cancer, some experienced laparoscopic surgeons have tried to avoid minilaparotomy. Totally laparoscopic gastrectomy (TLG) for gastric cancer requires that all procedures are completed using only laparoscopic devices, without requiring an additional minilaparotomy for specimen resection and anastomosis. Since this type of surgery results in less pain and faster recovery relative to open or laparoscopy-assisted surgery, which require laparotomy, more surgeons prefer it.5,6 However, an obstacle to TLG for gastric cancer is tumor localization without direct visualization or palpation to determine the area of stomach resection.
Tumor absence at the resection margin is very important to achieve success in gastric cancer surgery. If there is tumor involvement at the resection margin during distal gastrectomy for gastric cancer, additional resection is needed to avoid leaving tumor in the remaining stomach. In open and laparoscopy-assisted surgery, the primary tumor can be easily localized by palpation or direct visualization through a temporary gastrostomy. However, because the primary tumor is not easily detected in the laparoscopic view, determining resection margins for EGC in TLG may be difficult.
Several other methods requiring additional preoperative gastroscopy to determine resection margins have been proposed.7-9 However, additional preoperative gastroscopy can be inconvenient for patients. We introduced a procedure for intraoperative gastroscopy to directly mark tumors during TLG for gastric cancer in the middle third of the stomach.
In this protocol, we applied the laparoscopic surgery for patients with early gastric cancers at the preoperative studies, which are not included in absolute indication for endoscopic submucosal dissection.

intraoperative gastroscopy for tumor localization in laparoscopic surgery for gastric adenocarcinoma

Determining resection margins for gastric cancer, which are not exposed to the serosal surface of the stomach, is the most important procedure during totally laparoscopic gastrectomy (TLG). The aim of this protocol is to introduce a procedure for intraoperative gastroscopy, in order to directly mark tumors during TLG for gastric cancer in the middle third of the stomach. Patients who were diagnosed with adenocarcinoma in the middle third of the stomach were enrolled in this case series. Before surgery, additional gastroscopy for tumor localization is not performed. Under general anesthesia, laparoscopic mobilization of the stomach is performed first. After the first portion of the duodenum is mobilized from the pancreas and clamped, the surgeon moves to the other side for the gastroscopic procedure. On the insertion of a gastroscope through the oral cavity into the stomach, 2 - 3 cc of indigo carmine is administered via an endoscopic injector into the gastric muscle layer at the proximal margin of the stomach. The location of stained serosa in the laparoscopic view is used to guide distal subtotal gastrectomy, however, total gastrectomy is performed if the tumor is too close to the esophagogastric junction. A specimen is sampled after distal gastrectomy to confirm sufficient length from resection margin to tumor before reconstruction. In our case series, all patients had tumor-free margins and required no additional resection. There was no morbidity related to the gastroscopic procedure, and the time required for the procedure has gradually decreased to about five minutes. Intraoperative gastroscopy for tumor localization is an accurate and tolerated method for gastric cancer patients undergoing totally laparoscopic distal gastrectomy.

Of 20 patients who underwent intraoperative gastroscopy, 18 underwent distal subtotal gastrectomy, guided by findings at gastroscopy. However, we performed total gastrectomy on two patients, because the proximal tumor margin was too close to the esophagogastric junction, as determined on intraoperative gastroscopy. The distance from the tumor to the proximal resection margin was 3.5 cm and 2.5 cm in these two patients, respectively (Table 1).
Total mean operative time for distal gastrectomy was 188 min. The mean time for tumor localization with intraoperative gastroscopy was 8.4 min, and it gradually decreased from 11.8 min (initial 5 cases) to 4.6 min (last 5 cases) (Figure 5). Intracorporeal reconstruction with Billroth II anastomosis followed distal gastrectomy in all patients. There was no morbidity, except for gastric stasis and adhesive ileus in two patients. Finally, there were no complications related to intraoperative gastroscopy.
Mean tumor size in patients who underwent distal gastrectomy was 2.89 (range: 0.5 - 3.2) cm. EGC was the final diagnosis in 16 patients, but one exhibited muscle invasion and another exhibited serosal invasion. The mean distance from tumor to resection margin was 3.3 (range: 0.5 - 6.7) cm. Most common histologic type was signet ring cell carcinoma (Table 1).

Figure 1: Laparoscopic Clamp to Prevent Gas Passage into the Small Bowel. Before insertion of the gastroscope during surgery, the duodenum or jejunum should be clamped with a laparoscopic instrument. Gastroscopic procedures inevitably require the insufflation of gas into the lumen of the bowel to precisely visualize the lesions, and gas that has passed into the small bowel cannot be easily aspirated when the gastroscopic procedure finishes. Multispectral Real-time Fluorescence Imaging for Intraoperative Detection of the Sentinel Lymph Node in Gynecologic Oncology

The prognosis in virtually all solid tumors depends on the presence or absence of lymph node metastases.1-3 Surgical treatment most often combines radical excision of the tumor with a full lymphadenectomy in the drainage area of the tumor. However, removal of lymph nodes is associated with increased morbidity due to infection, wound breakdown and lymphedema.4,5 As an alternative, the sentinel lymph node procedure (SLN) was developed several decades ago to detect the first draining lymph node from the tumor.6 In case of lymphogenic dissemination, the SLN is the first lymph node that is affected (Figure 1). Hence, if the SLN does not contain metastases, downstream lymph nodes will also be free from tumor metastases and need not to be removed. The SLN procedure is part of the treatment for many tumor types, like breast cancer and melanoma, but also for cancer of the vulva and cervix.7 The current standard methodology for SLN-detection is by peritumoral injection of radiocolloid one day prior to surgery, and a colored dye intraoperatively. Disadvantages of the procedure in cervical and vulvar cancer are multiple injections in the genital area, leading to increased psychological distress for the patient, and the use of radioactive colloid.
Multispectral fluorescence imaging is an emerging imaging modality that can be applied intraoperatively without the need for injection of radiocolloid. For intraoperative fluorescence imaging, two components are needed: a fluorescent agent and a quantitative optical system for intraoperative imaging. As a fluorophore we have used indocyanine green (ICG). ICG has been used for many decades to assess cardiac function, cerebral perfusion and liver perfusion.8 It is an inert drug with a safe pharmaco-biological profile. When excited at around 750 nm, it emits light in the near-infrared spectrum around 800 nm. A custom-made multispectral fluorescence imaging camera system was used.9.
The aim of this video article is to demonstrate the detection of the SLN using intraoperative fluorescence imaging in patients with cervical and vulvar cancer. Fluorescence imaging is used in conjunction with the standard procedure, consisting of radiocolloid and a blue dye. In the future, intraoperative fluorescence imaging might replace the current method and is also easily transferable to other indications like breast cancer and melanoma.

Fluorescence imaging is a promising innovative modality for image-guided surgery in surgical oncology. In this video we describe the technical procedure for detection of the sentinel lymph node using fluorescence imaging as showcased in gynecologic oncologicy. Low-Cost Single-Port (LoCoSP) Device for a Transcervical Approach in Minimally Invasive Transhiatal Esophagectomy

Esophagectomy remains the preferred option to achieve curative treatment in advanced esophageal cancer, but the choice of surgical approach remains controversial. A transthoracic approach may improve lymph node dissection, but it has considerable morbidity and respiratory complications. Transhiatal access has been demonstrated as an efficient means of minimizing post-operative complications. Minimally invasive transhiatal esophagectomy may reduce operative trauma and morbidity as well as enhance postoperative recovery with no compromise in cancer recurrence or survival. Nevertheless, it has a technical limitation in terms of cervical esophagus dissection. Thus, a low-cost single-port device was developed to complete upper mediastinal dissection by a transcervical approach during minimally invasive transhiatal esophagectomy. This device uses a nasogastric tube, a number-eight sterile glove, a sterile sponge, and 3 permanent 5-mm trocars. The step-by-step process of transhiatal esophagectomy and the development of this device are described. This technique allows for the dissection of the upper mediastinum, as well as the esophagus over the aortic arch and behind the superior portion of the trachea. The harvesting of lymph nodes along the left recurrent laryngeal nerve and paratracheals was improved.

Laparoscopy-endoscopy Cooperative Surgery for the Treatment of Gastric Gastrointestinal Stromal Tumors

In view of the shortcomings of endoscopic or laparoscopic surgeries alone in the treatment of gastric gastrointestinal stromal tumors (G-GISTs), this approach makes an innovative improvement in the treatment of G-GISTs that are less than 5 cm in size. Laparoscopy-endoscopy cooperative surgery (LECS) is used to combine endoscopic and laparoscopic surgeries, fully realizing their respective advantages and avoiding their drawbacks. The main steps are as follows. First, gastroscopy and laparoscopy are combined to confirm the location and boundary of the tumor. Tumor resection is carried out laparoscopically, guided by a gastroscope. The specimen is removed orally and the gastric wound closed laparoscopically. Then, gastroscopy and laparoscopy are combined to determine whether there is wound bleeding, if the suture is satisfactory, and if the gastric cavity is deformed.
LECS has natural advantages in the treatment of G-GISTs that are less than 5 cm in size. The accurate estimation of tumor location and boundary greatly improves the complete resection rate of tumors. The risk of tumor rupture is substantially reduced, and the long-term prognosis of patients is significantly improved. The process allows for accurate resection of the tumor, maximum preservation of normal gastric tissue and organ function, and avoids postoperative gastric deformation. The patient's postoperative rehabilitation is greatly accelerated, and oral feeding can resume on the day of the operation. The specimen is taken out through the mouth to avoid the need for an extended abdominal incision. This greatly reduces the patient's postoperative pain and scarring. The method greatly shortens the postoperative hospital stay (i.e., discharge is possible on the day after the operation), increasing the turnover of hospital beds.

Laparoscopy-endoscopy cooperative surgery is appropriate for the treatment of gastric gastrointestinal stromal tumors less than 5 cm in size. It can achieve the respective advantages of endoscopic and laparoscopic surgeries while avoiding their drawbacks.

Clinical Application of Single-Surgeon, Three-Port, Laparoscopic Resection for Colorectal Cancer with Natural Orifice Specimen Extraction

Reduced-port laparoscopic surgery (RPLS) has been widely used for the radical resection of gastrointestinal tumors. Single-surgeon, three-port, laparoscopic radical resection for sigmoid colon or high rectal cancer with natural orifice specimen extraction surgery (NOSES) has the advantage of a small incision, quick postoperative recovery, and short hospital stay. Yet, there are still only a few reports on NOSES. This paper describes the indications, preoperative preparations, surgical steps, and precautions for single-surgeon, three-port, laparoscopic radical resection of the sigmoid colon and high rectal cancer, and intraoperative specimen collection through the natural orifice. 
The protocol focuses on the steps of radical dissection and the main technical points of resection and reconstruction. At the same time, a procedure for fixing an anvil seat by self-traction of extracorporeal silk thread, used for purse-string suture fixation after the proximal anvil was placed in the abdominal cavity, was creatively improved. This operation could effectively avoid problems such as an insufficient proximal intestinal tube, shaking off the anvil seat, and weak purse-string suture during a single operation. The surgical care had less variability and was easy to perform, effectively avoiding postoperative anastomotic leakage and bleeding due to excessive intraoperative anastomotic tissue. This surgery could be widely promoted in primary hospitals.

Modified Laparoscopic Anatomic Hepatectomy: Two-Surgeon Technique Combined with the Simple Extracorporeal Pringle Maneuver

Laparoscopic anatomic hepatectomy (LAH) has become increasingly prevalent worldwide in recent years. However, LAH remains a challenging procedure due to the anatomical characteristics of the liver, with intraoperative hemorrhage being a primary concern. Intraoperative blood loss is the leading cause of conversion to open surgery; therefore, effective management of bleeding and hemostasis is crucial for a successful LAH.
The two-surgeon technique is proposed as an alternative to the traditional single-surgeon approach, with potential benefits in reducing intraoperative bleeding during laparoscopic hepatectomy. However, there remains a lack of evidence to determine which mode of the two-surgeon technique yields superior patient outcomes. Besides, to our knowledge, the LAH technique, which involves the use of a cavitron ultrasonic surgical aspirator (CUSA) by the primary surgeon while an ultrasonic dissector by the second surgeon, has been rarely reported before.
Herein, we present a modified, two-surgeon LAH technique, wherein one surgeon employs a CUSA while the other uses an ultrasonic dissector. This technique is combined with a simple extracorporeal Pringle maneuver and low central venous pressure (CVP) approach. In this modified technique, the primary and secondary surgeons utilize a laparoscopic CUSA and an ultrasonic dissectorconcurrently to achieve precise and expeditious hepatectomy. A simple extracorporeal Pringle maneuver, combined with the maintenance of low CVP, is employed to regulate the hepatic inflow and outflow in order to minimize intraoperative bleeding. This approach facilitates the attainment of a dry and clean operative field, which allows for the precise ligation and dissection of blood vessels and bile ducts. The modified LAH procedure is simpler and safer due to its effective control over bleeding as well as the seamless transition between the roles of primary and secondary surgeons. It holds great promise for future clinical applications.

Techniques of Laparoscopic Right Posterior Sectionectomy: Glissonian Approach and a Parenchymal Transection Technique

Laparoscopic liver resections (LLR) have been widely accepted as a treatment option for liver tumors. They offer several advantages over open liver resections, including less blood loss, reduced wound pain, and shorter hospital stays with a comparable oncological outcome. However, laparoscopic resection of lesions in the right posterior section of the liver is challenging due to difficulties in bleeding control and visualizing the surgical field. In the past, laparoscopic right posterior sectionectomy (LRPS) was still in the exploration phase, with undefined risks in the Second International Consensus Conference on LLR in 2014. However, recent technological advancements and increased surgical experience have shown that LRPS can be safe and feasible. It has been found to reduce hospital stay and blood loss compared to open surgery. This manuscript aims to provide a detailed description of the steps involved in LRPS. Transumbilical Endoscopic Resection of Intra-abdominal Mesenchymal Tumor

Gastrointestinal stromal tumors (GISTs) typically occur in the stomach and proximal small intestine but can also be found in any other part of the digestive tract, including the abdominal cavity, albeit rarely. In the present case, the tumor was resected endoscopically through the anterior gastric wall. Computed tomography (CT) scan and gastroscopy of a 60-year-old woman revealed submucosal lesions in the gastric body. The possibility of a stromal tumor was considered more likely. The endoscopic surgery was performed under endotracheal anesthesia. After a solution had been injected at the lesion site in the stomach, the entire gastric wall was dissected to expose the tumor. As the lesion was in the abdominal cavity and its base was attached to the abdominal wall, it was accessed using a sterilized PCF colonoscope. A sodium chloride injection was administered at the base. The tumor was then peeled along its boundaries using the hooking and excision knife combined with the precutting knife. Subsequently, the tumor was pulled into the stomach through the incision made in the stomach and then extracted externally through the upper digestive tract using the ERCP spiral mesh basket. After confirming the absence of bleeding at the incision site, the endoscope was returned to the stomach, and the stomach opening was closed using purse-string sutures. The patient recovered satisfactorily following the surgery and was discharged on day 4. Histological examination revealed a low-risk stromal tumor (spindle cell type, &lt;5 mitosis/50 high-power fields [HPF]). Immunohistochemistry revealed positive staining for CD34 and CD117, negative staining for SMA, positive staining for DOG1, and negative staining for S100. Additionally, the expression of ki67 was 3%.

Single-incision plus one-port laparoscopic proximal gastrectomy with double-channel anastomosis (SILT-DT) is a minimally invasive surgical approach for treating proximal gastric cancer. This technique includes comprehensive laparoscopic resection of the proximal stomach, lymph node dissection, and double-tract anastomosis. By integrating single-port laparoscopic surgery with an auxiliary operating hole, SILT-DT reduces procedural difficulty while facilitating the placement of an abdominal drainage tube. Compared to the traditional five-port laparoscopic gastrectomy, SILT-DT requires fewer ports and results in shorter incision lengths, contributing to reduced postoperative pain and faster recovery. A thorough preoperative evaluation was conducted to ensure procedural success, with factors such as tumor size, stage, location, and patient BMI carefully considered. This comprehensive assessment allowed for optimal patient selection and surgical planning. Postoperative follow-up demonstrated no significant complications, underscoring the safety and efficacy of SILT-DT. This innovative technique offers a promising minimally invasive alternative for managing proximal gastric cancer while maintaining excellent clinical outcomes.

Robotics in Surgery: A Modular Robotic Platform Driven Gastric Wedge Resection

Robotic-assisted surgery has become increasingly popular since the introduction of the first robotic platform. Recently, a modular robotic system was approved for in-human use in Europe. Possible applications for this new robotic system are being explored, and standardized approaches are evolving. In lieu of this, a gastric wedge resection and the standardized setup for upper gastrointestinal procedures using this new system are presented here. This safe and feasible robotic procedure is demonstrated in a 69-year-old patient with a gastric tumor. All steps of the surgery are described in a detailed and reproducible manner. The article also details trocar positioning, arm adjustments, and required surgical instruments. Docking time amounted to 13 min, whereas the console time took 115 min. The patient was discharged after 4 days after ensuring an uneventful course. The presented method is also suitable for other surgical purposes, such as fundoplications or hiatoplasties, and ensures both generalizability and reproducibility.

Gastroscope-assisted Trans-Trocar Gastrostomy and Abdominal Exploration

Advanced endoscopic technologies have revolutionized gastrointestinal surgical procedures, transitioning from open or laparoscopic methods to endoscopic interventions. Gastrostomy is a classic clinical technique for patients with swallowing impediments due to obstructions in the cardiac or esophageal regions and has evolved significantly with the establishment of percutaneous endoscopic gastrostomy (PEG). While PEG offers reduced trauma and swift recovery, it falls short of exploratory capabilities and risks damaging adjacent organs. Therefore, we developed an innovative approach-gastroscope-assisted trans-Trocar gastrostomy (GTTG), which not only takes full play to the advantages of PEG with less trauma and faster postoperative recovery but also takes into account the advantages of laparoscopic gastrostomy with a good exploration of the abdominal cavity. This technique is characterized by ease of operation, minimal trauma (including short operation time and no redundant incision), rapid recovery, the ability to explore the abdominal cavity, and avoidance of injury to adjacent organs, which is quite worthy of widespread promotion in clinical settings.
